It is a hard one. It is kind of a double insult. It is saying both that she's ugly and two faced. It is somehow more insulting to say it this way than to call someone an ugly two-faced shrew. It is kind of mean, but all snark is kind of mean, just not this mean.

Well, yes. I'm not sure why that one seems mean, rather than just snarky. Plus it doesn't really make sense.
